The following forms should be completed and submitted to the relevant Returning Officer at the time of the submission of nomination form or before the close of the nomination period (i.e. on or before 04 August 2025) -
- Intention to Display Election Advertisements at Designated Spots [REO/C/20/2025ECSS(By-E)];
- Input Form for Introduction to Candidates - Candidates can only submit the Input Form for Introduction to Candidates provided by the Registration and Electoral Office. The form is enclosed in the nomination form pack. Any Input Form for Introduction to Candidates reproduced by candidates will not be accepted.

The above-mentioned forms may be obtained from District Offices, the offices of the Returning Officers or from the offices of the Registration and Electoral Office at 8/F, Treasury Building, 3 Tonkin Street West, Cheung Sha Wan, Kowloon or Unit 2301-03, 23/F, Millennium City 6, 392 Kwun Tong Road, Kwun Tong, Kowloon.
