Elections

A candidate who submitted a paper Input Form may download the above electronic version of Input Form (“e-Input Form”) for producing the text version of the Introduction to Candidates for voters and authorized representatives with visual impairment. Please fill in the candidate’s name and complete Part II of the e-Input Form only, and submit the duly completed e-Input Form to the Registration and Electoral Office by email or upload the relevant e-Input Form to the e-Form Upload Platform or Central e-Form Services by 5:00 pm on 17 November 2025.

The following forms should be completed and submitted to the relevant Returning Officer at the time of the submission of nomination form or before the close of the nomination period (i.e. on or before 6 November 2025) –

- Request by a Candidate for a Legislative Council Geographical Constituency, a Legislative Council Functional Constituency or a District Council Constituency to print Particulars relating to the Candidate on a Ballot Paper [REO/BP/7/2025(SF)] (if applicable);
- Consent Form of a Prescribed Body in Relation to the Request by a Candidate to print its Particulars as Particulars relating to the Candidate on a Ballot Paper [REO/BP/9/2025(SF)] (if applicable);
- Intention to Display Election Advertisements at Designated Spots [REO/C/20/LC(2025)];
- Input Form for Introduction to Candidates - Candidates can only submit the Input Form for Introduction to Candidates provided by the Registration and Electoral Office. The form is enclosed in the nomination form pack. Any Input Form for Introduction to Candidates reproduced by candidates will not be accepted.

The above-mentioned forms may be obtained from District Offices, the offices of the Returning Officers or from the offices of the Registration and Electoral Office at 8/F, Treasury Building, 3 Tonkin Street West, Cheung Sha Wan, Kowloon or Unit 2301-03, 23/F, Millennium City 6, 392 Kwun Tong Road, Kwun Tong, Kowloon.
