* The form “Intention to Display Election Advertisements at Designated Spots [REO/C/20/2026ECSSE]” should be completed and submitted to the relevant Returning Officer at the time of the submission of nomination form or before the close of the nomination period (i.e. by 5:00 pm, 20 October 2026).

+ Within 3 days after the end of Nomination Period (by 5:00 pm, 23 October 2026), submission of either the Electronic Input Form for Introduction to Candidates (“e-Input Form”) or the Input Form for Introduction to Candidates (“Paper Input Form”) is required. If candidates choose to submit Paper Input Form, candidates can only submit the Paper Input Form provided by the Registration and Electoral Office. The form is enclosed in the nomination form pack and can be obtained from District Offices, the offices of the Returning Officers or from the offices of the Registration and Electoral Office at 8/F, Treasury Building, 3 Tonkin Street West, Cheung Sha Wan, Kowloon or Unit 2301-03, 23/F, Millennium City 6, 392 Kwun Tong Road, Kwun Tong, Kowloon. Any photocopied / modified Input Form for Introduction to Candidates reproduced by candidates will not be accepted.

A candidate who has submitted a paper Input Form may download the above Electronic Input Form for Introduction to Candidates (“e-Input Form”) for producing the text version of the Introduction to Candidates for voters and authorized representatives with visual impairment. Please fill in the candidate’s name and complete Part II of the e-Input Form only, and submit the duly completed e-Input Form to the Registration and Electoral Office by email or via the e-Form Upload Platform by 5:00 pm, 4 November 2026.

The above-mentioned forms may be obtained from District Offices, the offices of the Returning Officers or from the offices of the Registration and Electoral Office at 8/F, Treasury Building, 3 Tonkin Street West, Cheung Sha Wan, Kowloon or Unit 2301-03, 23/F, Millennium City 6, 392 Kwun Tong Road, Kwun Tong, Kowloon.
